RESUMEN

OBJECTIVE: To estimate the prevalence of polypharmacy, describe the pharmacotherapeutic classes used, and investigate whether polypharmacy is associated with demographic and socioeconomic indicators, regardless of the number of diseases, among participants in the Brazilian Longitudinal Study of Adult Health (ELSA-Brasil) baseline (2008-2010). METHOD: In this analysis, 14,523 adults and elderly (35-74 years) participated. Polypharmacy was characterized as regular use of five or more medicines. The demographic and socioeconomic indicators analyzed were: gender, age, education level, per capita family income, and access to private health insurance. The independent association between demographic and economic indicators and polypharmacy was estimated by binary logistic regression. RESULTS: The prevalence of polypharmacy was 11.7%. The most used drugs were those with action on the cardiovascular system. After adjustments, including by number of diseases, the chances of being on polypharmacy treatment were significantly higher among women, older participants and those with greatest number of diseases. Individuals without health insurance had lower chance to be under polypharmacy, as well as those with lower income. CONCLUSION: The occurrence of polypharmacy among ELSA-Brasil baseline participants was mainly due to drugs for the treatment of chronic diseases. The relation between polypharmacy and the female gender, as well as its association with old age, are in consonance with the results obtained in other studies. Despite the absence of an association between polypharmacy and education level, the income and health insurance results reinforce the existence of social inequalities regarding drug use.

RESUMEN

OBJECTIVE: To characterize storage and disposal practices associated with expired medicines in home pharmacies of Primary Care users. METHODS: Cross-sectional study based on data collected from 423 users of 15 Primary Care units located in a Brazilian city, between August 2014 and July 2016. Data were collected via face-to-face interviews. Categorical (demographic and socioeconomic characteristics) and continuous variables were expressed as proportions and means and standard deviations, respectively . Storage behaviors and disposal practices associated with unused and expired medicines were described as frequencies. RESULTS: Most (83%) interviewees were female and approximately 70% had completed high school. The kitchen was the most common medicine storage place (58.6%). Approximately 75% of participants reported inappropriate medicine disposal practices. CONCLUSION: This study revealed high rates of inappropriate medicine disposal practices with direct impacts on pharmacological treatment and the environment. Continuing education of healthcare professionals and the general public is required to raise awareness about proper medicine use and disposal.

(AU)

Objective: To estimate the percentage of reticulocyte counts in relation to blood count performed in the clinical analysis laboratories in Divinópolis / MG. Methods: This is a descriptive study carried out in the clinical analysis laboratories of Divinópolis/MG registered in the National Register of Health Establishment (CNES). A questionnaire interview was conducted in order to obtain information on the number of blood counts and reticulocyte counts performed during the last twelve months. The questionnaire also aimed to identify whether the institution was public or private, whether it was outpatient or hospital care and which methods were used to perform the tests. Results: Of the 15 laboratories registered in the CNES, 11 were interviewed (73%). 276,666 blood counts and 4,248 reticulocyte counts were performed in the last 12 months in all laboratories, which means the percentage of reticulocyte count of 1.5% in relation to the blood counts performed. Most of the laboratories are private (81%), medium-sized (54%) and carry out outpatient care (54%). To perform the blood count, most (81%) use the flow cytometry technique and for reticulocyte counting all perform the manual technique with the bright blue cresyl reagent. Conclusions: The percentage of reticulocyte counts in relation to blood counts performed in laboratories in the city of Divinópolis/MG is low, even though it is the first complementary exam for the differential diagnosis of anemia. This low request may be related to the devaluation of the reticulocyte in the clinical environment or the lack of accuracy and poor reproducibility of the results found. (AU)

RESUMEN

Obesity, diabetes and hypertension are risk factors for cardiovascular diseases (CVD) because they promote a state of hypercoagulability. It is known that platelets play an important role in the development of atherosclerosis. Recent studies have evaluated platelet volume indexes (PVIs) in individuals with risk factors for CVD to better understand the platelet mechanisms involved in their development. The IVPs indirectly estimate platelet function and are easily obtained from automated hematology analyzers, which provide platelet counts, mean platelet volume (MPV), platelet distribution width (PDW) and the platelet-large cell ratio (P-LCR). The present study aims to review literature studies that investigated the association between PVIs and obesity, diabetes, and arterial hypertension, in order to evaluate its use as a potential subclinical marker of CVD. Studies have shown promising results for MPV, an index that allows for early detection of platelet activation and may be useful in identifying patients before the onset of CVD development so that preventive strategies can be implemented. The PDW, although evaluated by a smaller number of studies, also showed promising results. However, there is still a long way to go in order for the MPV and PDW to be used in clinical practice, since there is still a need for more epidemiological evidence, establishing reference values, and standardizing the way results are presented.

This work assessed the concurrent and face validity of the MacArthur scale, which attempts to capture subjective social status in society, neighborhood and work contexts. The study population comprised a convenience sample made up of 159 adult participants of the ELSA-Brasil cohort study conducted in Minas Gerais between 2012 and 2014. The analysis was conducted drawing on Conceptual Metaphor Theory and using corpus linguistic methods. Concurrent validity was shown to be moderate for the society ladder (Kappaw = 0.55) and good for the neighborhood (Kappaw = 0.60) and work (Kappaw = 0,67) ladders. Face validity indicated that the MacArthur scale really captures subjective social status across indicators of socioeconomic position, thus confirming that it is a valuable tool for the study of social inequalities in health Brazil.

Abstract This study analyzed data from the telephone-based Surveillance System of Risk and Protective Factors for Chronic Non-Communicable Diseases (VIGITEL) to assess the prevalence of clusters of healthy behavior patterns (non-smoker, non-alcohol-drinker, regular leisure-time physical activity and recommended consumption of fruit and vegetables) and the temporal trend between 2008 and 2013. Additionally, we evaluated whether the association between level of schooling and clustering of three or more healthy behavior patterns decreased in this same period. Prevalence ratios were obtained using Poisson regression. We found that between 2008 and 2013, the clustering prevalence of three or more healthy behavior patterns increased from 20% to 25% in men, and from 26% to 32% in women, suggesting an increase in the prevalence of healthy behavior patterns in Brazil. This increase was found at all levels of schooling. However, the association between levels of schooling and the prevalence of clustering of three or more healthy behavior patterns remained constant during the period. Thus, the results suggest that educational disparities in clustering of three or more healthy behavior patterns did not change over time, despite the social improvements observed in the country in recent years.

RESUMEN

OBJECTIVE: To present the recruitment and communication strategies of the ELSA-Brasil (Estudo Longitudinal de Saúde do Adulto - Brazilian Longitudinal Study for Adult Health). METHODS: The strategies were directed at dissemination, institutionalization and recruitment. The communication actions intended to promote the strengthening of a positive institutional image for the study, knowledge management and an effective dialogue with its target audience. An official website was created in order to communicate with different audiences, to disseminate scientific knowledge, and to contribute to consolidate the image of the study within society. RESULTS: We recruited 16,435 men and women, active employees and retirees of six public institutions of education and research, to constitute the cohort of 15,105 participants. The recruitment goals were fully achieved in the six centers, with a slight predominance of women and of younger adults, and slightly fewer employees with lower level of schooling. CONCLUSIONS: The strategies used were adequate and essential to the successful inclusion and participation of the employees.

RESUMEN

The debate about ethics in research with human beings has historically emphasized experimental studies because of their greater potential to harm the subjects involved. However, observational studies also include risks and relevant questions to be discussed. This article aims to present and discuss the ethical aspects involved in the implementation of ELSA-Brasil, a longitudinal multicenter study, with public funding, in which the research subjects and investigators are employees of the same institutions. The procedures adopted to meet the ethical requirements and commitments are described, as well as the casuistics that guided the actions according to their guiding principles (beneficence, autonomy and social justice). We present some moral problems that required consideration of risks and benefits at the confluence with the study's objectives, and we conclude with comments on the peculiarities and the potential benefits of a longitudinal study.

OBJECTIVE: To evaluate the prevalence of risk factors for cardiovascular disease in Japanese-Brazilian subjects. SUBJECTS AND METHODS: One hundred thirty-one residents of the Mombuca community were studied. Statistical analysis was based on the X² test, Fisher's Exact test, Student's t test, and ANOVA, at a 5% significance level. RESULTS: The average age was 56.7 years-old; 76.3% had dyslipidemia, 24.4% pre-diabetes (PDM), 10.7% type 2 diabetes mellitus (T2DM), 46.6% hypertension, 52.7% abdominal obesity, and 35.8% metabolic syndrome (MS). There were significant correlations between HOMA-IR and MS diagnosis and obesity, while HOMA-β levels were decreased in T2DM and PDM. The ankle-brachial index was positive for peripheral artery disease in 22.3% of the individuals. Electrocardiograms did not show increased evidence of myocardial ischemia. CONCLUSION: Subjects of this community are exposed to major cardiovascular risk factors, namely high prevalence of MS diagnoses and increased HOMA-IR. Arq Bras Endocrinol Metab. 2012;56(9):608-13.

There were significant correlations between HOMA-IR and MS diagnosis and obesity, while HOMA-ß levels were decreased in T2DM and PDM.

RESUMEN

OBJECTIVE: To estimate the prevalence of type 2 diabetes mellitus (DM2) and impaired glucose tolerance (IGT) in the Japanese-Brazilian community of Mombuca in relation to risk factors (FR) for diabetes and cardiovascular disease. SUBJECTS AND METHODS: Cross-sectional study with 131 individuals of Japanese ancestry (69% first generation), aged 20 years or more, submitted to socio-cultural, anthropometric, and biochemistry evaluation. RESULTS: Of the individuals studied (n = 131, age = 55.1 ± 15.9 years), 58.8% were women. The prevalence of DM2 and IGT were 13.7% and 14.5%, respectively. Regarding RF, 76.3% presented dyslipidemia, 52.7% abdominal obesity, 48.1% arterial hypertension, and 42.3% whole body obesity. CONCLUSIONS: The prevalence of DM2, IGT, and RF in this Japanese-Brazilian community was higher than in the adult population of the city of Ribeirão Preto-SP, Brazil, suggesting an increase in risk conditions for these morbidities.

OBJECTIVE: To analyze the association between chronic diseases and health risk behaviors and self-perceived health status by gender. METHODS: A total of 39.821 adults (30+ years old) who participated in the system Vigilância de Fatores de Risco e Proteção para Doenças Crônicas por Inquérito Telefônico (VIGITEL--Telephone-Based Surveillance of Risk and Protective Factors for Chronic Diseases) carried out in 27 Brazilian capitals in 2006 were included in the study. The dependent variable was medical diagnosis reporting of diabetes, hypertension and myocardial infarct or stroke. Respondents were grouped into three categories: no disease; one chronic disease; and two or more. The associations between the dependent variable and sociodemographic characteristics, behavioral risk factors (smoking, consumption of fatty meat and whole milk, leisure-time physical inactivity, low fruit and vegetable intake and intake of added salt) and self-perceived health status were assessed in men and women using multinomial logistic regression. RESULTS: Chronic disease reporting was higher among older men and women with lower schooling, BMI > or = 30 kg/m2 and who were on a diet. There was an inverse association between number of risk behaviors and two or more chronic diseases (OR: 0.64; 95% CI: 0.54;0.76 among men and OR: 0.86; 95% CI: 0.77;0.97 among women). Those men (OR: 33.61; 95% CI: 15.70;71.93) and women (OR: 13.02; 95% CI: 6.86; 24.73) who self-perceived their health as poor reported more chronic diseases. There was no statistical interaction between self-perceived health status and gender. CONCLUSIONS: An inverse association between number of risk behaviors and reporting of two or more chronic diseases suggests a reverse causality and/or higher survival rates among those who take better care of themselves. Men seem to have poorer perception of their health status compared to women, after adjustment for confounders.
